Introduction of Microbiology Reporting Templates is collected to reduce the time and similarity for beneficiaries. This is a collection of templates regarding branches of microbiology like bacteriology, mycology, parasitology, virology, and immunology but most commonly bacteriology.

Culture Negative Reports

  1. Urine Culture and Sensitivity:  No growth after 24 hours of incubation at 37°C.
  2. Pus/ sterile body fluids Culture & Sensitivity: No growth after 48 hours of incubation at 37°C.
  3. Blood Culture & Sensitivity: No growth after 5 days of incubation at 37°C.
  4. Sputum Culture & Sensitivity: No pathogen isolated after 48 hours of incubation at 37°C.
  5. Stool Culture & Sensitivity: No enteric pathogen isolated.

Pneumococcus/Streptococcus pneumoniae Panel: Oxacillin (screening drug, do not report), 0floxacin, Levofloxacin, Chloramphenicol, Erythromycin, Azithromycin, Doxycycline, Vancomycin, Clindamycin, Co-trimoxazole, and Linezolid.

Streptococcus species- viridans group-Penicillin (MIC), Ampicillin, Cefotaxime, Ceftriaxone, Vancomycin, Azithromycin, Erythromycin, Tetracycline, Levofloxacin, 0floxacin, Chloramphenicol, Clindamycin, Linezolid, and Meropenem (MIC).

Note: Please correlate clinically. It is preferred mainly in rare pathogens and normal flora involving as a pathogen in immunocompromized host/ patient.

Contamination, please repeat the mid-stream urine (MSU) sample.

Sputum for Gram Stain

Microbiology Reporting Templates  for Mycology

  • Sputum for KOH Mount: Fungal elements not seen
  • Sputum for KOH Mount: Yeast cells, yeast cells with budding, and Pseudohyphae seen
  • Candida albicans isolated
  • Candida species other than albicans isolated.


  • Please correlate clinically and if possible resend the specimen for clarification on whether the organism is really a clinical concern or contaminated.
  • Urine culture is only a quantitative (semi) method and resting all specimens are a qualitative method of culture.
  • Contamination, please repeat mid-stream urine (MSU) sample is requested when mixed growth of microbes on the culture plate of urine inoculated medium from the first urine sample of the patient.
  • Qualitative method culture is divided into three groups light, moderate and heavy growth of organisms.
  • The most commonly used antimicrobial disks are-Penicillin, Ampicillin, Ciprofloxacin, Ofloxacin, Cotrimoxazole, Gentamycin, Chloramphenicol, Nitrofurantoin, Cefoxitin, Teicoplanin, Erythromycin, Doxycycline, Levofloxacin, Clindamycin, Linezolid, Vancomycin (disk and E-strip), Ceftriaxone, Cefotaxime, Cefixime, Azithromycin, Tetracycline, Nalidixic acid, Meropenem, Amikacin, Aztreonam, Ampicillin sulbactam, Ampicillin clanuvic acid, Penicillin tazobactam, Ceftazidime, Cefepime, Imipenem, Tobramycin, Colistin (E-strip), Polymyxin B (E-strip), Colistin E strip and Oxacillin.
