Liver Cancer: An Overview of Introduction, Types, Pathogenesis, Laboratory Diagnosis, Treatment, Prevention, and Keynotes

Introduction

Liver cancer is a major global health problem and ranks among the leading causes of cancer-related mortality worldwide. It primarily arises from hepatocytes or bile duct epithelial cells and is frequently associated with chronic liver disease and cirrhosis. The most common form is primary liver cancer, though the liver is also a frequent site of metastatic tumors from other organs.

Types of Liver Cancer

Liver cancers are classified into primary and secondary (metastatic) tumors.

Primary Liver Cancers

  1. Hepatocellular carcinoma (HCC)
    • Most common type
    • Originates from hepatocytes
    • Strongly associated with cirrhosis and chronic hepatitis
  2. Cholangiocarcinoma
    • Cancer of the bile duct epithelium
    • Intrahepatic or extrahepatic
  3. Hepatoblastoma
    • Rare, occurs mainly in children

Secondary (Metastatic) Liver Cancer

  • Spread from the colon, pancreas, lung, breast, or stomach

Pathogenesis

Liver cancer develops through chronic inflammation, cellular injury, and regeneration, leading to malignant transformation.

  • Chronic hepatitis B and C infections
  • Liver cirrhosis due to alcohol or non-alcoholic fatty liver disease
  • Exposure to aflatoxins
  • Genetic mutations affecting cell cycle regulation
  • Persistent hepatocyte damage → fibrosis → cirrhosis → carcinoma
  • Angiogenesis and immune evasion promote tumor progression

Laboratory Diagnosis

Specimens

  • Blood
  • Liver biopsy or resected tissue

Diagnostic methods

  • Serum alpha-fetoprotein (AFP) – elevated in HCC
  • Liver function tests (LFTs)
  • Histopathological examination – confirmatory
  • Immunohistochemistry (HepPar-1, AFP, CK7/CK19)
  • Molecular testing in selected cases

Imaging support (clinical correlation)

Treatment

Treatment depends on tumor stage, liver function, and patient condition.

  • Surgical resection (early-stage disease)
  • Liver transplantation (selected cases)
  • Local ablation therapies (radiofrequency, microwave)
  • Transarterial chemoembolization (TACE)
  • Systemic therapy (targeted therapy, immunotherapy)
  • Palliative care in advanced disease

Prevention

  • Hepatitis B vaccination
  • Early detection and treatment of hepatitis B and C
  • Avoid excessive alcohol consumption
  • Control metabolic risk factors (obesity, diabetes)
  • Safe food storage to prevent aflatoxin exposure
  • Regular surveillance of high-risk individuals

Keynotes

  • HCC is the most common primary liver cancer.
  • Strongly linked to cirrhosis and chronic hepatitis
  • AFP is an important tumor marker.
  • Early-stage disease is potentially curable.
  • Imaging plus biopsy confirms diagnosis.
  • Prevention through vaccination and liver disease control is crucial.
