Coccidioides-Antigen Testing: Introduction, Purpose, How it works, When it’s used, Types of antigen tests, Limitation, and More

Introduction

Coccidioides antigen testing, used to diagnose coccidioidomycosis (also known as valley fever),  detects the presence of fungal antigens in bodily fluids like urine, serum, or cerebrospinal fluid. This aids in faster and more accurate diagnosis, especially in immunocompromised individuals or those with severe disease. 

Purpose

To detect the presence of Coccidioides fungal antigens in bodily fluids, indicating an active infection. 

How it works

Antigen tests, like the MiraVista Diagnostics MVista® Coccidioides Quantitative Ag EIA, look for specific fungal proteins (antigens) produced by the Coccidioides fungus. 

When it’s used

  • Faster Diagnosis: Antigen testing can lead to quicker diagnosis than traditional antibody testing.
  • Immunocompromised Patients: It’s particularly useful in immunocompromised individuals where antibody responses might be delayed or weak.
  • Severe or Disseminated Disease: Antigen testing can be helpful in cases of severe or disseminated coccidioidomycosis.
  • Meningitis: Antigen detection in cerebrospinal fluid (CSF) is recommended for suspected coccidioidal meningitis.
  • Monitoring Treatment: Antigen tests can help monitor treatment effectiveness and detect relapses.

Types of Antigen Tests

  • Enzyme Immunoassay (EIA): A common method for detecting antigens in serum, urine, or CSF.
  • Quantitative EIA: Measures the amount of antigen present, which can help monitor treatment response.

Complementary to Antibody Testing

Antigen testing is often used alongside antibody tests (e.g., EIA, immunodiffusion, and complement fixation) for a more comprehensive diagnosis. 

Limitations

  • Specimen Types:
    • Serum: Blood samples are commonly used for antigen testing.
    • Urine: Urine samples can also be used for antigen detection.
    • Cerebrospinal Fluid (CSF): CSF is crucial for diagnosing coccidioidal meningitis.
    • Bronchoalveolar Lavage (BAL): BAL fluid can be used in certain cases.
