The NT-proBNP (N-terminal pro B-type Natriuretic Peptide) test is a vital cardiac biomarker blood test used primarily to screen for, diagnose, and monitor heart failure. NT-proBNP is a physiologically inactive prohormone fragment released into the bloodstream alongside active BNP when the heart walls stretch or experience volume and pressure overload. Because of its long half-life (1–2 hours) and structural stability, it serves as an excellent diagnostic marker in clinical laboratories.
Sandwich Complex Formation: The patient’s blood sample is mixed with a biotinylated monoclonal antibody specific to NT-proBNP and another NT-proBNP antibody labeled with a ruthenium complex (Ru²⁺). They bind to form a sandwich complex.
Magnetic Capture: Streptavidin-coated microparticles are added to bind the biotinylated antibody complexes. The entire combination is then magnetically locked onto an electrode plate while unbound excess components are rinsed away.
Chemiluminescent Reaction: An electric voltage is applied to the electrode, initiating a chemical reaction that causes the ruthenium complex to emit light. A photomultiplier tube measures this light emission, and the intensity corresponds directly to the concentration of NT-proBNP in the sample.
Procedure

The Biotin Hook: High doses of biotin supplements (vitamin B7, commonly over 5 mg/day for hair and nail growth) can interfere with the streptavidin-biotin architecture used in ECLIA systems, leading to falsely depressed results. Patients must stop high-dose biotin therapy for at least 8 hours before blood collection.
Kidney Function Impact: Unlike active BNP, which is broken down by cellular receptors, NT-proBNP relies completely on the kidneys for clearance. Reduced renal function (chronic kidney disease) will naturally elevate baseline NT-proBNP readings regardless of cardiac strain.
