Introduction

RBC cast, or red blood cell cast, is a type of urinary cast containing red blood cells embedded within a protein matrix. They are a strong indicator of bleeding within the kidney and are commonly associated with glomerular diseases, such as glomerulonephritis. Its presence in urine sediment suggests damage to the glomeruli or tubules, and it can be a key diagnostic finding for various kidney conditions. 

Formation of RBC Cast

RBC casts form when red blood cells are trapped within a solidified protein matrix (primarily Tamm-Horsfall protein) in the kidney tubules. 

Appearance of RBC Cast

It can vary in appearance, with some being densely packed with red blood cells and others having fewer cells embedded in the protein matrix. The color can range from pink to red to brown.

Clinical Significance of RBC Cast

The presence of RBC casts is often associated with: 

  • Glomerulonephritis: Inflammation of the glomeruli, the filtering units of the kidney. 
  • Vasculitis: Inflammation of blood vessels, including those in the kidney. 
  • Acute Interstitial Nephritis: Inflammation of the kidney tissue between the tubules. 
  • Renal Infarction: Blockage of blood flow to the kidney. 
  • Other kidney diseases: RBC casts can also be seen in other conditions like lupus nephritis, subacute bacterial endocarditis, and even severe pyelonephritis. 

Contrast with other Casts

RBC casts differ from other types of casts, such as WBC casts (containing white blood cells) or epithelial cell casts (containing kidney tubular cells), which may indicate different kidney problems. 

Importance of microscopic examination:

RBC cast is typically identified through microscopic examination of urine sediment. 

Not always diagnostic

While RBC casts are strongly suggestive of glomerular disease, they can also be seen in other conditions, and their absence does not rule out kidney disease. 

Keynotes

  • RBC casts will be seen in the following conditions as vasculitis, inflammation of blood vessels; acute Interstitial nephritis, renal Infarction, blockage of blood flow to the kidney; diabetic nephropathy; and tubular injury.
  • RBC casts form within the renal tubules when RBCs are trapped within a matrix of Tamm-Horsfall protein.
  • RBC casts are often associated with glomerular bleeding and are a hallmark of glomerulonephritis, vasculitis, and other conditions affecting the glomeruli.
  • They can vary in appearance; some casts are densely packed with RBCs, while others may have fewer RBCs within a hyaline or granular matrix.
  • RBC casts are fragile and can disintegrate easily if the urine sample is not examined promptly.
  • The presence of dysmorphic RBCs (abnormally shaped RBCs, like acanthocytes) alongside RBC casts further strengthens the suspicion of glomerular hematuria.
  • RBC casts, especially when accompanied by dysmorphic RBCs, are considered a strong indicator of glomerular hematuria, which can aid in diagnosing and monitoring glomerular diseases like IgA nephropathy, ANCA-associated vasculitis, and lupus nephritis.
