India Ink Smear is a direct microscopic technique used for the rapid identification of encapsulated fungi, particularly Cryptococcus neoformans and Cryptococcus gattii, in clinical specimens such as cerebrospinal fluid (CSF), sputum, and urine.

This simple, rapid, and cost-effective test allows visualization of the thick polysaccharide capsule surrounding the yeast cells, which appears as a clear halo against a dark background.
Uses
- Primary diagnostic test for Cryptococcus infections, especially cryptococcal meningitis.
- Performed on CSF, sputum, bronchial washings, and exudates to detect encapsulated yeast cells.
- Helps differentiate encapsulated yeasts (Cryptococcus) from non-encapsulated yeasts (Candida).
- Provides immediate presumptive diagnosis before culture or antigen testing.
- Useful in resource-limited settings as a rapid screening test for fungal meningitis.
Principle
- India ink (nigrosin) acts as a negative stain, staining the background dark.
- The capsule of Cryptococcus does not take up the stain, appearing as a clear, refractile halo around the yeast cell.
- The central cell stains faintly, while the capsule remains unstained, making the organism highly distinctive.
Keynotes
- The capsule appears as a bright halo surrounding round yeast cells (4–10 µm).
- A positive India ink test is highly suggestive of Cryptococcus neoformans/gattii.
- Sensitivity: ~50–80% (higher in HIV-infected patients due to higher organism load).
- Limitations: False negatives in low fungal burden; antigen tests (CrAg) are more sensitive.
- Always confirm with fungal culture or cryptococcal antigen detection (CrAg test).
- India Ink Smear remains an important bedside diagnostic tool for early detection of cryptococcal meningitis in resource-constrained regions.
