Rothia kristinae, Granulicatella elegans, and Gardnerella vaginalis are all Gram-positive (or Gram-variable) bacteria that exist as part of the human normal microbial flora, but they differ significantly in their taxonomy, physiological characteristics, and clinical roles.

Keynotes
Rothia kristinae
- Taxonomy: Formerly classified under the genus Kocuria (Kocuria kristinae) before being reclassified into Rothia.
- Characteristics: It forms pale cream to orange colonies on agar and tests catalase-positive and coagulase-negative. It is frequently mistaken in clinical labs for coagulase-negative staphylococci.
- Pathology: Usually a harmless skin and mucosal commensal, it acts as an emerging opportunistic pathogen capable of causing bacteremia, line infections, and endocarditis in severely ill or immunocompromised patients.
Granulicatella elegans
- Taxonomy: Previously categorized as nutritionally variant streptococci (NVS) or under the genus Abiotrophia.
- Characteristics: Highly fastidious, requiring specific growth supplements like L-cysteine or pyridoxal (vitamin B6) to grow well on standard media. This growth difficulty contributes to culture-negative diagnostic scenarios.
- Pathology: Resident of the oral and mucosal flora. It is infamous for causing severe, destructive cases of infective endocarditis, as well as joint, bone, and central nervous system infections.
Gardnerella vaginalis
- Taxonomy: A distinct rod-shaped bacterium belonging to the family Bifidobacteriaceae.
- Characteristics: It is Gram-variable, meaning it can stain either positive or negative due to a thin cell wall. It is non-spore-forming and thrives in the human vaginal ecosystem.
- Pathology: Unlike the other two species, which commonly cause invasive systemic infections (like endocarditis or bacteremia), Gardnerella vaginalis is primarily associated with mucosal dysbiosis. Overgrowth in synergy with other anaerobic bacteria leads to bacterial vaginosis, characterized by characteristic “clue cells” on microscopic wet mount examination.
